Diabetic Neuropathy, Neuropathic Pain and Marijuana (Cannabis): An Alternative Treatment

As medical director for Medical Cannabis of Southern California I often evaluate patients who suffer from neuropathic pain as a result of diabetes.  Neuropathic pain means pain that is associated with direct pathology to the nerves themselves.   This is in contrast to pain in a muscle whose signal is transported to the brain via the healthy nerves.   In diabetes the elevated levels of sugar in the blood actually cause damage to the nerves leading to chronic pain.   Cannabis has been shown in several studies to reduce the pain associated with various causes of neuropathic pain including HIV neuropathy.

Typically diabetic neuropathy starts by affecting the peripheral nerves to patients feet and legs.  They can experience sharp pain when walking, decrease sensitivity to temperature, tingling, numbness and eventually when the nerves because completely destroyed no feeling at all.  I once took care of an elderly women with severe diabetic neuropathy who fractured her ankle to the point where her tibia was protruding directly through her skin.  Basically it snapped in half.  Amazingly she felt NO PAIN because her nerves were destroyed.

Many patients get put on medications such as gabapentin (neurontin), narcotics like vicodin and even anti-depressants such as amitryptiline.    Most patients report very little relief in their pain but also have unwanted side effects.   They report a significant reduction in their pain with even small doses of cannabis.   They can use it as often as they need without any fear of overdose, addiction or end organ damage.  They also can medicate using a digital vaporizer which makes it odorless and smoke free!

Marijuana, sex and your marriage

Over the past month I have had more than a few patients tell me that in addition to helping them with their symptoms, that their sex life has improved dramatically as a result of their cannabis use.    Many couples get into a “rut” when it comes to sex.   It becomes less and less frequent and even with couples that have sex regularly it can become “not as exciting”.     What they described is that while they are medicated the sex becomes more intense, they tend to be less inhibited and frequency has gone up.

When it comes to quality of life and your marriage it’s hard to think of too may things that can improve a marriage then a good sex life.    Couples that routinely have sex are 75% more likely to stay married.  That is a huge quality of life factor.

I am beginning to talk more and more about introducing cannabis into relationships to improve couples sex lives.   I truly believe that this will become a much more common use of the medication over time.

Marijuana Evaluations in Long Beach

My name is Dr Sean Breen and for the past year in Long Beach I have dedicated my practice to seeing patients for medical marijuana recommendations.  Our clinic, Medical Cannabis of Southern California, is located next to Cal State Long Beach in the Los Altos Medical Plaza.  I want to take a second to thank my entire staff for working so hard to reach out to our community here in Orange County.  We have seen almost 5000 patients and have done good work.

Here are some things to consider when deciding on what clinic (doctor) you are going t o receive your recommendation from.

1. Do not use a clinic where you are seen by a physician assistant (PA) or Nurse Practitioner.  You must have your recommendation signed by a licensed physician (MD/DO).  These clinics that are being run by PA’s are slowly being investigated and shut down.  Your recommendation will no longer be able to be verified and you have throw away your money.

2. Do not go to a clinic that has ANY affiliation or sits adjacent or even in a dispensary.  Many clinics are advertising $45 recommendations because they are being run by dispensaries who know that you are going to spend money in their store.  The problem is that if the dispensary gets raided the clinic will most likely be shut down as well.  AGAIN YOUR REC WILL NO LONGER BE VERIFIABLE

3. Do not fall for the $60 recommendation fee.  They charge that and then the doctor only gives you a 6 month recommendation and charges you to renew.  The problem is that now you have to take another day to go see the doctor for the same thing and pay all over again. THEY WILL TELL YOU ITS 12 MONTHS OVER THE PHONE…. BUT WHEN YOU SEE THE DOCTOR HE HAS A CHANGE OF HEART.  ITS CLASSIC BAIT AND SWITCH.

4. STAY AWAY FROM CLINICS THAT GIVE YOU A RECOMMENDATION BUT DO NOT REQUEST YOUR MEDICAL RECORDS! By law they have to have medical records on file.   Clinics like this will not be able to back up their recommendations in court and the doctor can potentially lose his license to practice.

Lymphoma and Medical Marijuana: Pray for Naya!

Today in Irvine I had a 25 year old patient by the name of Naya.   The reason why she is coming in is because a few weeks ago she was diagnosed with Hodgkin’s Lymphoma, a cancer that effects the white blood cells called Lymphocytes.   She is going to be starting chemotherapy shortly and is using cannabis for the nausea, pain and lack of appetite that comes from chemotherapy treatment.   Cannabis is an amazing medication for cancer patients because it is the ONLY thing that will eliminate the nausea and improve a patients appetite.    Unfortunately Naya is also 6 weeks pregnant and at the time of my appointment was losing her pregnancy due to her illness.   So on top of dealing with a new diagnosis of cancer she is grieving the loss of her first child.  Naya’s husband is a combat veteran of Iraq who suffers from PTSD and also uses cannabis to treat his symptoms.

With all that said I told her I would post a blog so that we can collective pray for healing, pray for peace and pray for their child.

Naya and Nathan: I pray that God brings healing into your life.  I pray that he helps you cure this cancer and bring peace to you and Nathan during this difficult time.  I pray for your unborn child.  I pray that his or her sole lives in eternity with God.  Be blessed today.  Thank you for allowing me to take care of you.

Trust Pharmaceutical Companies? Read on.

As physicians we take an oath that states: “First do no harm.” Unfortunately as doctors we have to many times rely on data that has been produced and evaluated by drug companies that stand to make HUGE profits from the sale of these drugs.  A 32 billion dollar per year company, named Pfizer, is just one of many that knowingly distributed a pill that was harmful to patients.   It is for this reason that so many patients DO NOT WANT TO TAKE PILLS and rely on a more natural remedy like cannabis.

In 2002 Pfizer was AGGRESSIVELY pressing its sales reps to push doctors to prescribe a drug by the name of BEXTRA to patients suffering from acute pain.   It was an off label indication for the drug.

Drug reps were given huge sales bonuses for selling doctors that this drug was safe to give for patients for chronic arthritis, pre menstrual syndrome (labeled indications) and also acute pain (an off label indication)

BEXTRA is a drug classified as a COX-2 inhibitor.  It was used to stop inflammation in conditions like arthritis and was promoted because it DID NOT cause stomach ulceration and GI bleeding like many NSAIDS (non steroidal anti-inflammatory drugs)  such as motrin.

AMAZINGLY, there were studies conducted that CLEARLY showed that BEXTRA increased the risk of HEART ATTACK and STROKE in high risk patients (ie older patients with arthritis and OH YEAH heart disease).  PFIZER KNEW ABOUT THESE STUDIES AND IGNORED THEM!!!!! ARE YOU KIDDING ME!

THANKS TO DR DENNIS MANGANO, who compiled his own data and presented it to the FDA after pfizer had already pushed through incorrect safety data, thousands of patients lives were saved.   He single handedly was responsible for this drug not only being taken off the market but pfizer ultimately being charged with a crime and paying out millions of dollars in damages.

It is a FACT that patients DIED AS A RESULT OF PFIZERS criminal negligence.  DOCTORS WERE LIED TO. PATIENTS WERE LIED TO. PATIENTS ULTIMATELY DIED.

PFIZER DESERVES A HUGE F U!

Its no wonder that so many patients are turning to medical cannabis to treat their pain. It is a plant given to us by God.   It is not toxic to any of our major organ systems.   There is never been an overdose from using cannabis.  There is no risk for physical addition.   Patients don’t have to worry about drug drug interactions.   And IT WORKS!  Patients report a huge reduction in pain, an increased ability to fall and stay asleep, relaxation and a reduction in muscle spasm.   Studies have shown that it clearly is a safe alternative for treating pain.

Alternative Treatment for IBS (Irritable Bowel Syndrome): marijuana (cannabis)

Today I evaluated a 26 year male who has suffered from intense abdominal cramping, constipation and intermittent diarrhea for the past 10 years.   He has been tentatively diagnosed with Irritable Bowel Syndrom (IBS). He has a significant family history for Crohns disease but has been unable to schedule a colonoscopy due to no insurance.  For the past 6 years he has been medicating with cannabis to control his symptoms.

Cannabis has a stabilizing affect on the digestive system.  Because it binds to the nerves that control the digestive system it will reduce symptoms of IBS.   Patients with diarrhea will start to have normal bowel movements.   Abdominal cramping will be less frequent and intense.   Patients appetites will improve.

Plaquenil, Rheumatoid Arthritis, Lupus and Marijuana (Cannabis)

As medical director of Medical Cannabis of Southern California I often evaluate patients who suffer from rheumatoid arthritis and SLE (lupus).  Both are characterized by extreme chronic joint pain.   Today I evaluated a 54 year female who had been taking plaquenil (hydroxychloroquine) to limit the joint inflammation.    Unfortunately she is experiencing some unwanted side effects and has been using cannabis to help relieve them.

Plaquenil is a drug that is used to treat malaria infections.   It also happens to have immunosuppressive properties and is used to limit inflammation.   Arthritis is a disease that is characterized by pain secondary to joint inflammation.

This patient stated that the plaquenil was working great and her joint pain had been reduced tremendously.  However it was causing pretty debilitating abdominal cramping, nausea and occasional vomiting.    In many cases this can be reduced by taking with food but for her it was persistent and frequent.

A few weeks back she ran into a friend who had been using cannabis to treat her lupus arthritis.   She convinced my patient to try a little to see if it would help.  Not surprisingly her abdominal cramping subsided and her nausea went away.   In addition her joint pain was significantly improved.

Cannabis has many anti-inflammatory properties.  It directly binds to receptors on immune cells and modulates the inflammatory response.   In addition it also reduces pain by directly binding to receptors in our nervous system.

My treatment plan for her is to try vaporizing an indica strain at night to help with sleep and use a topical cream during the day.  She will most likely use cannabis indefinitely as there is no cure for arthritis (outside a joint replacement)

From 18 oxycontin per day to 2 in 2 weeks!!

A week ago I blogged about a women by them name of Sam who had been taking 12 Norco and 6 Oxycontin daily for the past 5 years.   I was elated when after ONE WEEK she had decreased her opiate use to 4 Norco and 4 Oxycontin.

TWO WEEK UPDATE: Sam is down to ONLY 2 PILLS A DAY!!!! She takes one oxycontin in the morning and a norco in the afternoon.  I sat down with her and this is what she said.

“CANNABIS HAS SAVED MY LIFE!”  WOW!  LAST MONTH HER PCP WROTE HER A PRESCRIPTION FOR 600 OXYCONTIN!!!! INSANITY.

I realize that running this practice is NOT going to be a glaring star on my resume.  In fact many of my colleagues will shun me for being a part of this industry and this medicine.

BUT IT IS THE SAM’S OF THE WORLD THAT KEEP ME MOTIVATED AND REMIND ME THAT I AM DOING THE RIGHT THING!  WHAT A SUCCESS STORY.

Alternative Treatment for Alcoholism: Cannabis

Today I evaluated a 59 year old male patient who has been sober for the past 15 years.  He still routinely takes Antabuse 250 mg which is a drug that will cause severe nausea and vomiting if he consumes alcohol.   Unfortunately he developed permanent nerve damage to his right arm as a result of his Antabuse medication.  So he only uses it in emergency situations.

Fortunately he has been able to use cannabis when he comes home from work to relax and “unwind”.   He stated that is completely eliminates any thoughts of having a drink to relax.  He has been successfully doing this for the past 15 years.

He finally decided to become a legal patient so he did not have to purchase any medicine off the street.  I am extremely proud of him for his sobriety and glad that cannabis played an essential role in his continued recovery.

Medical Cannabis Patient Success! Chronic Pain!

Last week I evaluate a 61 year old women named Sam who had multiple medical problems.  Her main complaints were that she suffers from Parkinsons and has debilitating resting tremors and more importantly that she has been taking 18 pain pills EVERY DAY for the past 5 YEARS!!!  (12 of which were oxycontin)  They give her terrible constipation, make her feel terrible and only slightly relieve her pain.

She followed up with me this week and was SO HAPPY because she is down to only 8 pain pills per day in JUST ONE WEEK of using cannabis and is already starting to sleep much better.

OUR GOAL IS FOR HER TO BE COMPLETELY OFF ALL OPIATE NARCOTICS IN ONE MONTH!!!  I KNOW WE CAN DO IT.

It is for this reason that I get so excited about cannabis as a medicine and how much it is helping so many of my patients!
